The chief freelance epidemiologist of the Lipetsk Region Health Department Andrei Filatov told the “First Number” how self-isolation is different from quarantine, what it can give and when the effect of this difficult test for Lipans will be known.

– What is the difference between quarantine and self-isolation?

– Generally speaking, there is no such term as quarantine now. There is a legal term “restrictive measures”. In practice, it means “sitting and not going out.” Quarantine is a complete ban on moving. During quarantine, people are not advised to stay at home, but strictly forbidden to leave. As you know, during the cholera they cordoned off the city with troops and didn’t let anyone out. This is quarantine. And self-isolation (this term was coined by Sergei Sobyanin. – Approx. Ed.) Is when people are offered to voluntarily limit contacts with other people. These may be those who returned from abroad, from Italy, for example, from France, etc. Why? Because they could be in contact with potentially infected or already sick people and become a source of coronavirus infection. Self-isolation is offered to people over 65 years old, who suffer from chronic diseases, those who can perform their work remotely. This preventive measure is recommended to all those who went on a “long weekend” by presidential decree. However, after the decision of the regional administration on protecting the population from coronavirus infection came out, self-isolation for the majority of the population became a duty for which legal sanctions are provided. But if a person can receive a warning or pay a fine for not observing the regime of self-isolation, then a person who violates quarantine can be sent to prison. – The governor, referring to the experience of the countries of Southeast Asia, where the coronavirus was defeated, as I understand it, believes that self-isolation is one of the best ways to combat COVID-19. And what do you think?

“I think so.” Coronavirus infection is transmitted by airborne droplets in a chain from one person to another. To prevent transmission, these chains need to be broken. How can they be torn? It is necessary to make sure that people cease to communicate, so that they are isolated from each other. This is a radical but effective approach. Moreover, communication between people should be interrupted for a long time. Coronavirus infection is dangerous because its incubation period lasts 14 days. In influenza, for example, this period is up to five days. In other infections known to us, it is also short-lived. Therefore, self-isolation should last at least two weeks.

“You say self-isolation breaks the chains.” But there are many cases when, for example, the wife is in isolation, and the husband continues to work. He rides on buses, makes contact with people, and it is possible that sooner or later he will bring this infection to the house, because the chains you are talking about are not broken as securely as you would like.

– Nobody says that self-isolation is a panacea. Today we live in conditions of biological effects on humans of a scale that is difficult and impossible to stop overnight. When we pin our hopes on self-isolation, we idealize it a little and think that people will stay at home and will not go anywhere for 14 days. But life makes them move: someone goes to the store, someone continues to work. But this does not mean that self-isolation is not effective. We estimate the population effect of this event. That is, we take and look at what is happening precisely in the population, in society, and not in a particular unit of society. In a family that somehow has contacts with the outside world, someone may get sick, but most families are completely self-isolated, and in general, cases of infection with coronavirus should decline.

– How do you assess the attitude of Lipans to the regime of self-isolation?

– Well, you yourself see that on the streets, one might say, uninhabited. The first week of self-isolation was especially deserted. I think this is an indicator of high consciousness of citizens. Yes, there are some people who, as always, do not accept, protest, ignore, but they are few. However, this week there were noticeably more people on the streets. I think this is due to the fact that many enterprises that closed were allowed to work. I mean hairdressers, dry cleaners, notaries.

– Already (April 9th. – Ed.), Can any conclusions be drawn about the effectiveness of self-isolation?

– Not now. It’s too early now. I have already said that the incubation period of a coronavirus infection lasts 14 days. Accordingly, in order to understand whether or not that we have broken the chain of transmission of infection, we need to wait another five days. On April 7, as you know, the president met with virologists, he asked them the same question, and they gave him the same answer: “we can return to this question in a week, when the incubation period will pass after separation, and then it will be clear which trends, trends in incidence. ” Today we can judge the effectiveness of self-isolation only by the example of the countries of Southeast Asia.
